The Integrated Care Management (ICM) model equips providers with competencies to provide high quality healthcare. ICM Is a care delivery model designed to achieve the quadruple aim outcomes of better health, better care, at lower cost, restoring joy of work. ICM defines the requisite competencies, practices and tools needed to engage patients through the provision of person-centered, evidence-based, coordinated care. The ICM training program permits replication of the ICM model. Training focuses on practices to position your organization for success in a value-based reimbursement environment. ICM tenets are in alignment with the new 2018 CoPs by supporting the comprehensive assessment, care planning, care coordination process while maximizing patient rights, leading to high quality care. Topics include improving patient health literacy and staff communication, maximizing patient learning, motivational interviewing, action planning, and empathetic care delivery. Included are four computer based narrated, disease modules outlining evidence-based guidelines and self-management support for Heart Failure, Diabetes, COPD and Depression plus 4 stoplight (zone) tools. Training is provided via live WebEx interactive sessions designed to solidify competencies for successful model implementation.
- A comprehensive exam evaluates content mastery

- An ICM Trainer Certificate is awarded and endorsed by NAHC, valid for a 3 year period
